How to make an appointment as a New Patient
- Download and complete New Patient Intake form
and the Informed Consent From . Email or bring the completed forms to the doctor's office. Upon email receipt of the forms, you will receive a call from the office to schedule your first visit with Dr.Yarish. - Bring copies of all relevant lab reports from the past 3 years if available.
- Bring bottles/labels of supplements and pharmaceutical drugs you are taking and provide names and dosages of medications.
Telephone Consultations Telephone consultations are generally intended for follow-up consultation and clarification of treatment plans. Telephone consultations are offered to new patients only after an initial visit has been conducted and a treatment plan has been initiated. Voicemail messages will be returned generally on the same day within the business hours Monday thru Saturday, unless Dr. Yarish is on an urgent call or out of town. You must always call 911 or visit the emergency room if you have a medical emergency. Phone consultations are charged according to fee schedule Fees and Payment PolicyPayment is due at the time of service. We accept cash, debit cards and credit cards. Patients are provided a receipt of the visit and have the option of submitting it to their insurance company. We do not bill insurance companies. Most insurance companies in Ontario recognize naturopathic medical expenses. Cancellation PolicyThere is no charge for cancellations with 24 hour notice. A full office visit fee will be charged for failure to provide any notice of cancellation. Emergencies and Hospital CoverageDr. Yarish is a specialist in Naturopathic Medicine. He does not have hospital admitting privileges. It is strongly recommended that his patients have a working relationship with a physician who has hospital privileges and on-call coverage.
